Buongiorno Dott. Torcetta,
mi chiamo Massimo e sono il papà di una splendida bimba di nome Miriam nata prematura (leggi Il neonato prematuro) a 5 mesi e mezzo con un peso di 490 g.
Ora dopo 4 mesi pesa 2 kg 250 g.: le condizioni di salute sono eccelenti ma dispensa ancora di ossigeno forzato i cui valori del flusso sono 20/16 e ha una
displasia broncopolmonare.
In quanto tempo riuscirà a guarire e avrà ripercusioni in vita?

Caro papà,
Miriam ha avuto una bella costanza e grande tempra. Il peso e l’età gestazionale la classificano nella estrema prematurità con peso estremamente basso.
Attualmente neonati così prematuri sopravvivono e hanno una qualità di vita variabile a seconda del decorso post-natale. Purtroppo dobbiamo comunque pagare un prezzo a questo progresso nella neonatologia.
Il neonato prematuro, a questa età gestazionale, nasce con un polmone che ancora non è completamente sviluppato (foto a lato) e questo è il problema più grande al quale bisogna fare fronte. In caso di minaccia di parto pretermine viene somministrato il cortisone alla mamma per tentare di migliorare la situazione polmonare e, effettivamente, questo è stato uno dei momenti più importanti della neonatologia negli ultimi 40 anni.
Un’altra risorsa a nostra disposizione è il surfattante che è la sostanza che permette ai polmoni di rimanere aperti dopo i primi respiri.
Nonostante questo però ci troviamo di fronte a polmoni estremamente fragili. Negli ultimi anni le tecniche di ventilazione del neonato sono estremamente migliorate ma rimane il fatto che noi soffiamo aria nei polmoni con una pressione discreta e questo non avviene nella respirazione normale.
Inoltre usiamo ossigeno (sempre meno ormai) e questo è comunque dannoso per le cellule in crescita.
In conclusione, la Neonatologia ha fatto passi da gigante ma rimaniamo ancora piuttosto dannosi per neonati in via di sviluppo e terribilmente delicati.
La Displasia Broncopolmonare del neonato prematuro è il risultato di questi effetti dannosi. È una patologia praticamente costante nei neonati estremamente pretermine perchè non è evitabile.
Anche i piccoli che hanno necessitato di pochissimo supporto ventilatorio ne soffrono in misura variabile.
Accade che i polmoni dei piccoli vadano incontro a un danno da infiammazione che ne ritarda e cambia lo sviluppo provocando una riduzione degli alveoli e una certa fibrosi.
Questo si ripercuote clinicamente con la necessità di ossigeno per durata variabile e talvolta (in età più avanzata) con una maggiore sensibilità alle infezioni polmonari o allo spasmo bronchiale.
Non posso prevedere i tempi di svezzamento di Miriam dall’ossigeno ed è fortemente probabile che la piccola possa anche essere dimessa con ossigeno a domicilio. La prognosi è comunque buona anche se sarà possibile che nel primo anno di vita possa essere necessario ricorrere in particolari momenti (infezioni) all’ossigeno ma, successivamente, questo si risolverà.
In alcuni casi resta comunque una maggiore reattività bronchiale che predispone a fenomeni asmatici nell’infanzia.
Generalmente i neonati come Miriam vengono comunque monitorati anche per quanto riguarda la funzione polmonare.
